General

We are a group of doctors, nurses and therapists dedicated to bringing specialist care for our older Singaporeans to your doorstep. AgeWELL ARTSZ is the platform we hope to use to achieve this.

Our friendly staff are all trained in the care of the elderly person. Our team comprises a Consultant Geriatrician and nurses with many years of experience and training in gerontology and care for the older person.

Our therapists who also have many years of experience in the care for the older person.

Our Approach
As a team, we believe that as far as is possible, everyone should age gracefully in the comfort of their own homes with there loved ones close to them. Our aim is to facilitate this for those who wish to do so. Our approach is that of holistic care – not only do we look after the patient’s medical needs, we also look after their nursing and therapy needs. We strive to ensure that the caregiver is well equipped with the necessary knowledge and skills to provide optimum care to the patient.

We aim to provide holistic management on site at our clinic as well as within the patients’ homes.

  • Assessment and preparation of the Lasting Power of Attorney

Specialist Clinic

At AgeWELL Seniors Clinic, we provide comprehensive, holistic geriatric assessment in a community-based multidisciplinary specialist clinic setting.

This allows mobile elderly access to a comprehensive assessment by a multidisciplinary team of professionals in an out-patient setting. We have the added advantage with the clinic being sited within the community rather than in a hospital based setting; making it easily accessible.

Our multidisciplinary team comprises a geriatrician, nurses trained in gerontology and experienced therapists.Our clinic based services commenced on the 4th of July 2011.

At AgeWELL Seniors Clinic, we offer

Geriatric Clinic

  • Provides a full assessment and management of the elderly patient with problems such as dementia, falls and mobility problems, continence problems, nutritional problems.
  • We also see elderly patients with multiple medical problems, heart failure, Parkinson’s disease, stroke etc

A dementia clinic for patient with early dementia

  • This clinic is run by our occupational therapist and our nurses.
  • The main aim of this clinic is to assess deficits in memory and function and to attempt to rehabilitate the patients in a small group setting.
  • The programme includes an assessment by our geriatrician at the beginning and at the end of the programme.

A falls and balance clinic for mobile patients at risk of falls

  • This clinic is run by our physiotherapist.
  • The aim is to restore balance and improve mobility and confidence in patients who have experienced falls or are at risk of falls.
  • This clinic also includes a full assessment by our geriatrician.

Home Care Services

AgeWell ARTSZ provides home based medical, nursing and therapy services.

This enables elderly patients with poor mobility to continue to receive comprehensive medical, nursing and therapy services from within the home environment.

Home Medical

Elderly patients with multiple medical problems often have concomitant impaired mobility. As a result, it may be difficult for them to travel out of the home to access medical services.

AgeWELL ARTSZ Medical Group provides home based medical care provided by a geriatrician trained in the care of the elderly.

Some of the types of medical problems handled by a geriatrician are:

  • Dementia and its complications including behavioural problems, acute confusional states etc
  • Elderly who have experienced falls or are at risk of falls
  • Elderly with nutritional problems
  • Those with feeding problems
  • Patients with multiple medical problems with polypharmacy and its risks of adverse reactions
  • Elderly patients with continence problems and issues with constipation

Home Nursing

Similarly, our nurses who are also trained in the care of the elderly, provide home based nursing advice and follow-up for this group of patients. Nursing services provided are:

  • Changing of nasogastric feeding tubes
  • Changing of urinary catheters
  • Management of wounds and pressure ulcers
  • Management of constipation and urinary incontinence
  • Management of behavioural problems in patients with dementia
  • General review of the patient’s status
  • Medication management and training
  • Nutrition management and advice
  • Home based individualized caregiver training
  • Caregiver counseling and support

Home Therapy

Home based therapy involves an occupational therapist and physiotherapist visiting the elderly patient at home to provide rehabilitation support within the home environment.

Care Giver Training

We provide care giver training for care givers of the elderly (includes family members, domestic help, staff of other healthcare set-ups for the elderly).

This training is conducted by professionals trained in the care of the elderly.
